Attention all 2006 CCCF Parents:

 

This is a reminder of Saturday's upcoming Final Regular Registration. This
will be the last open registration for the 2006 season. Registrations after
Saturday will only be on an as-available basis. If you have not submitted
all of the paperwork below, please come to Registration this Saturday with
your missing information. As you can imagine, managing the paperwork for
registration is a large task and the sooner we can get it all in, the easier
it will be for the coaches, players, parents and board members. 

 

1.	Proof of residency showing the address for which you registered (we
need to retain a copy)
2.	Original birth certificate PLUS a copy to be retained
3.	Last year's report card 
4.	Physical Form completed and signed (the form is on the website under
the Key Information tab). All players must have completed Physical Forms
submitted before they can step on the practice field!
